Your use of MyChart is governed by the complete version of the MyChart Terms and Conditions. Before using MyChart, you must read and accept the complete version (opens in a new tab).

To use MyChart you must agree to these conditions:

    Age Restrictions

  • Right now, you must be at least 12 years old to sign up for MyChart.


  • You should NEVER use MyChart for emergencies. If you have an emergency call 911 or go to the nearest hospital emergency room.
  • All of the messages you send to your doctor are private. However, some messages may be sent to other staff members to help us respond to you more quickly.
  • We will try our best to respond to all messages within 3-5 business days.
  • Please do not use any obscene or threatening language in your messages. Your account may be closed if you do.
  • All messages sent through MyChart will be added to your medical record.

    Security and Privacy

  • We use many security features to make sure that your information is private.
  • Although we do not generally share your medical information with anyone without your permission, we may use and disclose your medical information, without your permission, to treat you, to receive payment for health care provided to you, for certain of our health care operations, or when required by law for public health activities, law enforcement, or other public benefit functions.
  • You should not share your username and password with anyone. If you do, that person can see your records.
  • You must log in to MyChart to see any messages from your health center. No medical information will be sent to your email address.

  • MyChart has links to other sites for medical information. Although we feel that these sites are trustworthy, we are not responsible for their information.
  • Neither these websites nor MyChart are able to make diagnoses or treat medical problems. This is only something your doctor can do.


  • MyChart does not contain your complete medical record. It is only a summary of your record. If you would like to see your complete record please contact the Medical Records Department at your health center.
  • If you believe the information in your MyChart health record is not correct, contact your provider or the Medical Records Department at the location where you received services.
  • We try our best to make sure that MyChart is always working properly but there may be times when the system is shut down. We are not to blame for any problems that may occur if you are unable to see your medical information.
  • You may also contact our MyChart Support Team Monday through Friday from 9:00 a.m. to 5:00 p.m. at 1-844-622-5465 or 1-925-370-5920 for MyChart related questions.

(Email is not a secure method of communication therefore do not send confidential information when requesting MyChart support.)
  • These terms and conditions can be changed. When they are changed you will be able to see the changes when you log in.
